$15,000 Term Life/ $15,000 Accidental Death & Dismemberment (AD&D) benefit is provided to plan primary members; $7,500 Term Life/$7,5002 AD&D for spouse and $2,5002 Term Life/ $2,5002 AD&D for dependent children are also provided (if applicable). The primary member and eligible dependents (if applicable) are covered by the Term Life/AD&D benefit both in the USA and if/while traveling outside the USA; there are no restrictions due to the duration/time period of the travel. The Term Life/AD&D benefit goes into effect the first (1st) of the month after ninety (90) days from the enrollment date.

The amounts of both the Term Life and AD&D are reduced at age sixty-five (65) and older for both the primary member and spouse, but not the dependent children.
Beginning on January 1st following the 65th birthday of the primary member, $15,000 Life/$15,000 AD&D amounts are reduced to:
from member’s 65th birthday to age 70
from member’s 70th birthday to age 75
from member’s 75th birthday to age 80
from member’s 80th birthday to age 852
Beginning on January 1st following the 65th birthday of the spouse, $7,500 Life/$7,500 AD&D amounts are reduced to:
from spouse’s 65th birthday to age 70
from spouse’s 70th birthday to age 75
from spouse’s 75th birthday to age 80
from spouse’s 80th birthday to age 852
These reductions do not, however, apply to or affect the $2,500 Term Life/$2,500 AD&D amounts of the dependent children.
Should the death of the primary member, spouse, or dependent children result from an accident, they are eligible to both the Term Life and AD&D benefit amounts; their Beneficiary may claim and collect the sum of both benefit amounts.
ACCIDENTAL DEATH & DISMEMBERMENT (AD&D) BENEFIT
The Insurance Company pays this benefit to the primary member and eligible dependents (if applicable) if they lose their life, limb, or sight due to an accident. All of the following conditions must be met:
1. The insured is covered for AD&D Insurance on the date of the accident.
2. The loss occurs within 180 days from the date of the accident.
3. The cause of the loss is not excluded (see EXCLUSIONS below).
The Insurance Company pays to the insured the benefit shown below if they suffer any of the losses listed. The Full Amount is shown on the Schedule of Benefits. The Insurance Company Pays only one Full Amount while the Group Policy is in effect. If the insured has a loss for which The Insurance Company paid ½ of the Full Amount, The Insurance Company pays no more than ½ of the Full Amount for the next loss.

Loss of hands or feet means being permanently, physically severed at or above the wrist or ankle. Loss of sight means total and permanent loss of sight.
The Insurance Company does not pay a benefit for loss of use of the hand or foot.
Death benefits are paid to beneficiary. All other benefits are paid to the primary member or eligible dependents (if applicable).
Submitting a Claim
The member or someone on their behalf must send to CalStar - Claims a written notice of the loss on which the claim will be based. The notice must –
1. Include information to identify the claimant, like name, address and Group Policy number,
2. Be sent to Calstar - Claims,
3. Be sent within 20 days after the loss for which claim is based has occurred or as soon as reasonably possible.
A written notice of the loss on which the claim will be based should be sent by the member or someone on their behalf to Calstar – Claims at the address or fax number shown below, within 20 days after the loss has occurred,

· Proof of loss claim forms will be sent by CalStar to the member, within 15 days after the receipt of the notice of claim.
· Completed Proof of loss claim forms must be returned to CalStar – Claims within 60 days after the loss.
· Payments are made directly to the beneficiary, insureds, or legal representative with an Explanation of Benefits (EOB). CalStar will receive a copy of the EOB, showing the date and amount of payments.
· Death Payments are made by the Insurance Company (ReliaStar/ING Life) within 60 days of the date of receipt of the completed proof of death by ReliaStar/ING Life.
Please note that for Death Claims (Accidental or otherwise), one claim should be submitted.

The Insurance Company does not pay benefits for loss directly or indirectly caused by any of the following:
1. Suicide or intentionally self-inflicted injury, while sane or insane,
2. Physical or mental illness.
3. Bacterial infection or bacterial poisoning, Exception: Infection from a cut or wound caused by an accident.
4. Riding in or descending from an aircraft as a pilot or crew member.
5. Injury suffered while in the military service for any country or government.
6. Injury which occurs when you commit or attempt to commit a crime.
7. Use of any drug, narcotic or hallucinogenic agent –
a. Unless prescribed by a doctor
b. Which is illegal
c. Not taken as directed by a doctor or the manufacturer.
$15K Life/$15K AD&D: Association group insurance benefit provided through an insurance policy (67432-0; effective July 1, 2011) issued and underwritten by ReliaStar Life Insurance Company, a member of ING. The Term Life/AD&D benefit is not available in Guam, Puerto Rico, and Virgin Islands.
All applicable benefit coverages, including those of the primary member, spouse, and dependent children, are terminated on January 1st following the primary member’s 85th birthday.
Provided the primary member is under age 85.
