Limitation for Eligible “Retail” Generic Medications
The maximum dosage per month is 30 days and a quantity of 100. This program allows a maximum of $1,200 per calendar year for each individual with a $300 limitation each quarter and allows a maximum of $2,400 per calendar year for a family program with a $600 limitation each quarter on generic medications at your local network pharmacy.
The PlanRx "retail" generic maximum does not include diabetic supplies, fertility agents, sexual dysfunction medications, injectables, anorexiants, smoking deterrent medications, hair replacement products, cosmetic alteration drugs, Retin-A for individuals over the age of 18, insulin syringes and accessories, non-prescription drugs, or refills in excess of the number authorized (100) or in excess of a month's supply.
Limitations for Brand Medications
The brands included under the prescription maximum are subject to change. It is our intent to include as many medications as possible. All medications not included under the maximum may be checked for preferred pricing. Brand medication does not include some controlled substances and is limited to the number of units prescribed per 30-day period. Brand medications are mail order only and generally are for a 90-day supply.
